- Undergird and support all our efforts in the advancement and application of chronic care management in clinical practice and real-world solutions
- Collaborate with experts across the healthcare continuum for research, education and leadership in demonstrating the impact of chronic care management on healthcare reform.
- Foster innovation in chronic care management practices and techniques in four key areas:
- Provider/Recipient relationships and trust-building
- Self-efficacy improvement and behavior change modification
- Evidence-based best practices
- Maximum utilization of technology
- Serve as a laboratory to test, implement and deploy solutions that benefit the business community
- Provide an ongoing forum for healthcare experts to refine existing models of care delivery and develop new models
- Offer regular conferences and educational sessions to widely disseminate knowledge of best practice chronic care management.
- Advocate for chronic care management as the starting point for healthcare reform
- Establish chronic care management as the foundation of a truly sustainable healthcare system in the 21st century
