Our current menu of programs and initiatives reflect our focus on improving chronic care management as the logical starting point for genuine healthcare reform. Poorly managed chronic disease is at the root of our current healthcare crisis, both in terms of personal health outcomes and the incredible financial burden for society. We not only believe that’s where we should start, we are doing something about it right now!
