Federal/State Initiatives

We are actively involved in efforts at the state and federal level to advance chronic care management as a first step in healthcare reform. We believe the homecare industry is an “army at the ready” to provide chronic care management since that is exactly what we’ve been doing for decades! Our model offers a solution to recalibrate and reinvigorate the homecare segment of healthcare, positioning it to play a strategic role in reform.

Money Follows the Person (MFP) is one example of government seeking a solution . It is a federal grant awarded to 31 states to safely return nursing home residents to their homes. We collaborated with Arkansas Medicaid to develop a program for our state that combines clinical expertise and innovative technology to meet the needs of this population. One of the most exciting components of our solution is Telerehab, the use of interactive video to augment face-to-face therapy visits with distance therapy and support. This tool is perfectly suited for rural areas where access to healthcare is often limited. We are in the first year of this 5-year project.

We are committed at 2020 Health Solutions to advocating legislative and payment system changes that will facilitate chronic care management. We also support the elevation of homecare to an integral role that supports physicians and their patients in managing chronic disease. We need and welcome your support in pursuing these initiatives.
