Palliative Care
Our model for the management of chronic disease emphasizes ongoing, trusting relationships between medical professionals and patients living with chronic conditions. We have frequent opportunities to identify the patients’ individual goals and work with them in reaching their goals. As chronic conditions progress toward the end of life, we talk with patients about their goals at that stage of the disease process. For example, it becomes a natural discussion about palliative care for pain and symptom management to improve the quality of life, or transfer to Hospice to receive the unique benefits of that program.
